Support Services

We provide a comprehensive system of services designed to reduce barriers, build foundations toward self-sufficiency and pave pathways out of homelessness. Starting with personalized case management, we offer a web of holistic wrap-around support services to address the whole person and their individual goals for success. We extend this comprehensive support to the clients we meet through all of our service offerings.

  • Intensive Case Management

    Our Case Managers are dedicated, hands-on coaches who work with local unhoused residents to resolve their homelessness by trying to identify and overcome the barriers they face. Our Case Management team help their clients set goals and work hand-in-hand with them to navigate each step along toward accessing shelter or housing. We ensure clients are connected to all the services they require—needs assessment, shelter referral, housing applications, job training and career search, educational services, financial literacy training, legal services, mental health and addiction treatment, vouchers for IDs and transportation, access to medical care, and more.

  • Healthcare for the Unhoused

    In partnership with Santa Clara Family Health Plan, our Supervising Community Health Worker connects our unhoused clients with Medi-Cal coverage, provides medical referrals and support, offers health education and advocates for their patient’s care.

    We also partner with Valley Medical Homeless Program and Gardner South County Health Center to bring a team of health professionals who are trained to work with unhoused patients to assess their needs, provide on-the-spot treatment for many issues, health consultations, and referrals including mental health care. These services are provided at our twice weekly Unhoused Popup sites.

  • Housing & Shelter

    Our case managers are trained to capture critical information to assess our client’s vulnerability through the VI-SPDAT (Vulnerability Index - Service Prioritization Decision Assistance Tool). By having each client complete this assessment, they are ranked according to several vulnerabilities, and placed in a queue for supportive housing that prioritizes those who are at the highest risk. As a result, an average of over 100 unhoused clients access permanent supportive housing each year saving our community around $8.3 million each year.

    We provide case management for local Armory Shelter clients, as well as unhoused and housing insecure students at Gavilan College, through two separate contracts.

    For those with access to monthly income, we assist with the housing search and the application process to find and secure housing that fits their needs and budget. We try to find creative approaches when more traditional avenues prove untenable. As a certified partner with Seasons of Sharing, and Housing Industry Fund (HIF), we’re able to provide funding for the up-front costs of renting, such as deposit assistance to qualifying applicants.

  • Employment

    We partner with Work2Future, and the Gavilan College Certificate program, to provide training, internship, job readiness, and job placement assistance for those who need it most.

    This has allowed many of our clients to resolve their homelessness, or eliminate the risks contributing to their housing insecurity.
